REFER A PATIENT

If you would like to refer your patient for either implant placements or any other oral surgery case please use the form below to provide the necessary information.  We will then contact your patient to arrange a consultation with our specilaist consultant Maxillofacial surgeon Dr Atheer Ujman.

Following the consultation a full treatment plan report will be provided to your patient and a copy will be sent to you for your records.
